I understand that I will be required to provide a copy of my driver’s license during the application process in order to confirm my identity.
I agree to a criminal background screening by Heartfelt Egg Donation, if required.
I understand that I am required to complete Medical and Psychological Screening once matched and contracted with prospective parents.
I understand that I will be required to perform self-injections of fertility medications for approximately 2 – 3 weeks.
I understand that I may be required to travel for 1-2 days for a medical evaluation and approximately 5 – 10 days for the egg retrieval procedure if selected and contracted with Prospective Parents whose physician is out of the area. All travel is within the United States.
I understand that I will be required to undergo a medical procedure under sedation to remove my eggs from my ovaries.
I understand that Heartfelt Egg Donation, LLC does not guarantee or promise that I will be matched with Prospective Parents or promise any time frame in which this selection will occur. I understand and acknowledge that there is a possibility that I may never be selected as an Egg Donor.
I understand that Heartfelt Egg Donation will not provide legal, medical, psychological or genetic advice.
I understand that Heartfelt Egg Donation, LLC has the right to deny my application without explanation.
I understand that the information provided in this Pre-screening application will become part of my full Egg Donor Profile upon acceptance.
I voluntarily consent to participate as an egg donor in the Heartfelt Egg Donation Program.
